We have had to make a few changes to our Pre-Registration packet so we ask all teams to please use the attached updated packet and disregard the previous one sent prior.

Changes/Additions had to me made in regards to Personal Injury which is now reflected on the Consent Form that all members have to SIGN when you submit your application.
